Madison County District Parent/Guardian:

 

We are now accepting instructional choice selection for the second semester of the 2020-2021 school year (January 19, 2021 through June 3, 2021.   Please complete the attached form and return in person or mail to the school board office or email to Heather Motes at [email protected].  We must receive your selection no later than November 30, 2020. 

 

If we do not receive a form from you designating your choice, we will accept that as you have not requested a change from your child’s current instructional choice and no changes will be made.

 

Please note that the choice you make now will remain in effect for the entire second semester (January 19, 2021 through June 3, 2021).
